Sports physicals
Clearance for football, soccer, basketball, cheerleading, wrestling and other sports. Along with the standard exam, a sports physical looks at musculoskeletal health and cardiovascular readiness.
Bring the physical form from your school's athletic department or your league. For Florida high school sports, that is the FHSAA preparticipation physical form. Fill in the parts meant for the parent and athlete before you come.
School and camp physicals
Florida requires a school-entry health exam, done within the year before enrolling, for children entering a Florida school for the first time. Many summer camps also ask for a current physical on file. Bring the form your school or camp gave you and we will complete it at the visit.
Adult, employment and wellness physicals
- Employment and pre-hire physicals
- Annual wellness exams, to know your numbers and stay ahead of problems
- Pre-surgery clearance
- College and university entrance exams
- General health screening for any reason
What happens during a physical
Dr. Wolfington reviews your medical history and any symptoms, then does a head-to-toe exam with vital signs (blood pressure, heart rate, temperature and oxygen level), height and weight, vision screening and reflexes. Any required forms are signed on the spot.
What to bring
- The form from your school, team, camp or employer, if you have one
- A photo ID and a list of current medications
- Glasses or contact lenses, for the vision screening
- For children under 18, a parent or guardian
